Students with incomplete packets will be placed on the Pre-K waiting list. w In order to be enrolled, children must be four pears of age on/or before September 1,2022. Register your child TODAY! Documents required on March 24 th : 1) Completed Pre-K Packet 2) Copy of Certified Birth Certificate 3) Copy of Social Security Card 4) Immunization Certificate Form 3231 5) Current Proof of Receiving Food Stamps, SSI, Medicaid, TANF (Mo EBT cards) 6) Proof of Jenkins County Residency (Utility bill with SERVICE ADDRESS. The bill must say “Service Address”, Cell phone bills are not acceptable. If the bill is not in the name of the child's parent/guardian, a notarized note from the person responsible for the bill verifying that the child lives at that address must be provided.) 7) Eye, Ear and Dental forms (EED forms) are due before the I st day of 2022-2023 school year.
