Kidney stones a painful experience
NEW ORLEANS
Some people who have experi
enced kidney stone colic, caused
by a kidney stone trying to pass
through the urinary system, say
the pain is the most intense they
have ever felt. Emergency room
physicians sometimes prescribe
morphine for this condition. Some
patients undergo lithotripsy, a
surgical procedure that breaks up
4he stone into bits that can pass
‘more easily through the patient’s
aireter. The Ochsner Stone Clinic
in New Orleans works with each
kidney stone patient to help him
or her avoid the painful experi
ence in the future by preventing
kidney stones from forming.
Louisiana and other Gulf South
states comprise what is known as
the “Stone Belt,” a region where
kidney stones occur at a higher
rate than other areas in the Unit
td States, according to Jill
Lindberg, M.D., director of the
Ochsner Stone Clinic in New Or
leans.

eessive amounts of calcium, can
form small calcifications or stones
in the kidneys. Dr. Lindberg says
there are many causes for kidney
stones. Part of the treatment is to
evaluatethe patient’s metabolism
to determine how kidney stones
can be prevented in the future.

“Akidney stone typically begins
as a tiny speck of solid material
deposited in the kidney, where
urine collects before leaving the
body,” says Dr. Lindberg, whois a
nephrologist. “As more material
clings to the first speck, it gradu
ally builds into a solid object. Cal
cium-related kidney stonesare the
most common form of stones.
Sometimesthey are the resultofa
genetic trait which causes a per
son to over-absorb calcium in the
dietortoleak calciumin theurine.
Another very common cause is
decreased production of a stone
inhibitor called citrate.”

(NAII) shows that 803 insurers
competed for Georgia’s personal
and commercial property-casu
alty insurance business in 1994.

to form stones around a crystal
“seed.” Levels of urine oxalate,
uric acid, urine citrate, oxalate
and Tamm-Horsfall protein ex
cretion are also measured. Re
search has shown correlations
between these materials in the
body and the formation of kidney
stones. The test is the result of
research collaboration among the
Ochsner Department of Urology,
the Section on Nephrology and
the Research Division.
According to Dr. Lindberg, a
combination of factors contributes
to the South’s designation as the
Stone Belt. These include hot
weather and a diet high in salt
and red meat.

cals in the urine.”
A high-salt diet also leads to
kidney stone disease because a
highlevel of sodium in the urineis
usually accompanied by a high
level of calcium, which increases
the risk of stone formation.
Kidney stones sometimes can be
avoided by a change in diet and/or
preventivemedication. Dr. Lindberg
notes that prevention is cost-effec
tive; the work-up and 24-hour urine
test cost one-fifth the amount of a
single lithotripsy procedure.
