Ga Pharmacist Always on Duty CHILDREN’S DENTAL HEALTH My twelve-year-old son re cently chipped a front tooth when he fell off his skate board. When I took him to my dentist, I was told that my son would have to wait for a few years before the tooth could be capped. Why can’t it be fixed now? At your son's age, the pulp of the tooth — containing the nerves and blood vessels — is quite large. Preparing it for a crown might injure the pulp and could kill the tooth. How ever, as a tooth ages — or in response to an injury — den tin is deposited on the pulpal walls and the pulp start? become smaller. Periodic x rays of the tooth will enable the dentist to determine the proper time for preparation without injury to the pulp. If a large portion of the tooth is broken off, the den tist may decide to perform endodontic treatment. This type of procedure — root ca nal therapy — is performed to save the tooth, and it is a more complex measure than placement of a simple crown. In this process, the dentist removes the injured pulp, treats the area to prevent fur ther infection or damage to the tooth structure and fills the tooth. The dentist may need to crown the treated tooth if too much of the structure is damaged. This procedure will serve to keep the tooth in the mouth for many years — perhaps a life time. When your son’s tooth is ready to be crowned, it will orobably be filed down and fitted with a con ventional jacket crown. This crown is made entirely of porcelain or acrylic resin and will match the natural color of the teeth. However, porcelain 1 or acrylic crowns are not strong enough far heavy chewing and may fracture. This is why this type of ma terial is not used on back teeth. I am going to have a tooth crowned. Would you tell me something about the kinds of crowns that are used? The porcelain or acrylic crown was described in my answer to the previous letter. The gold arown is usually used on molars. The appliance is made entirely of gold, is very strong and is made speci fically to bear the heavy chewing burden placed on the back teeth. Another type of crown is called the veneer crown. Ba sically, this is a gold crown which has a veneer of por celain bonded to surfaces that are visible when a person smiles or speaks. The veneer does not change the strength of the crown and is applied for cosmetic purposes. In this process, the pulp is removed from the tooth and a rod-like extension —a dow el — is fitted into the root portion of the tooth and ce mented in place. A false tooth is attached to the dowel and also cemented. Crowns are used when a tooth is badly damaged and an ordinary filling cannot do the job or will not remain tightly in place. If properly cared for, crown-type restora tions will provide years, or perhaps a lifetime, of service.
