above-named minor child to
JAVELYN DAVIS. The request
is made pursuant to O.C.G.A. §
15-11-240 and shows the Court
the following:

The Petitioner nominates
JAVELYN DAVIS who resides
at 1206 Walden Brook Drive,
Lithonia, Georgia, 30028, to be
appointed as the permanent
guardian of the minor child pur
suant to O.C.G.A. § 15-11-240.

The Mother, Jasmine Davis’
last known address is 2707
Cavalier Way, Decatur, Geor
gia 30034 however the mother
has since moved to an un
known address.
3.
The father of Jayla Davis is Jai
Cook. He is deceased.
4.
Child has been living with the
potential guardian since May
2022. Child reports Mother has
put her out the home several
times. Child also states Mother
would prevent child from at
tending school to stay home
and care of younger siblings.
This caused child to fall behind
in school. Child was solely re
sponsible for the day-to-day
care of her younger siblings.
5.
This Petition further alleges that
the minor child’s best interest
would be served by the grant
ing of the permanent guardian
ship as the proposed perman
ent guardian can provide a safe
and permanent home for the
children. The individual chosen
as the proposed guardian is the
individual most appropriate to
be the child’s permanent guard
ian taking into consideration the
best interest of the child. The
children lived with the pro
posed permanent guardian for
most of their lives and was do
ing very well in the home of the
proposed guardian, which has
provided the children with the
most stability. The proposed
guardian is able to continue to
offer the minor children stabil
ity and continue to care for their
needs.
WHEREFORE, the Petitioner
respectfully prays that, pursu
ant to O.C.G.A. § 15-11-240,
this Court appoint Joya Davis,
as the permanent guardian with
all the rights and duties
provided under O.C.G.A. § 29-
2-21, 29-2-22, and 29-2-23,
and administer him the oath re
quired of a guardian pursuant
to O.C.G.A. § 29-2-24.
