1.1 COMPETITIVE COST

tion of the Fort Yargo Raw
Water Project - Contract A
& B — will be received until
September 29, 2016 at 2 PM
at City Hall, 25 East Midland
Avenue, Winder, GA 30680, at
which time and place they will
be publicly opened and read
aloud. Any bid received after
said time and date will not be
considered by the Owner. No
bid may be withdrawn after
the closing time for the receipt
of bids for a period of sixty
(60) days except as specifi
cally may be provided in the
Instructions to Bidders and/or
by State law.
1.2 SCOPE OF WORK
Raw water intake structure,
raw water pump station, and
raw water transmission main.
The work shall consist of
furnishing all products and
performing all labor necessary
to perform the following:
CONTRACT A
Construction of a Raw
Water Intake Structure, ±170
LF 24” ball and socket ductile
iron raw water intake line, a
raw water pump station, com
plete with instrumentation and
controls, mechanical, electri
cal, miscellaneous piping and
valves, gravel paving, earth
work, grassing, erosion and
sediment control measures,
and all appurtenances to pro
vide a complete installation.
CONTRACT B
Construction of ± 30,000
LF of 24” raw water main,
complete with bores, fittings,
combination air valves, drive
way and road replacement,
grassing, erosion and sedi
ment control measures, and
all related work for a complete
installation.
All Work shall be completed
in accordance with the plans
and specifications. The Work
will be awarded in two (2) Con
tracts, one each for Contract
A& B.
Time allotted for completion
of work is 240 consecutive cal
endar days. All Work shall be
completed in accordance with
the plans and specifications.
The Work will be awarded in
two (2) Contracts.

1.4 CONTRACTOR
LICENSE
The Scope of Work as
described herein is defined as
“utility contracting” in accor
dance with O.C.G.A 43-14-
2(17) so the Contractor per
forming the work must provide
proof of a valid license by the
State of Georgia as a “Util
ity Contractor” and must also
employ a “Utility Manager”
certification holder who will
have oversight of all the util
ity work. Therefore, the Con
tractor submitting a bid must
list the license number of the
Contractor performing the util
ity contracting on the face of
the bid envelope and on the
bid form and provide proof of
a valid license with the bid.
No bid will be opened unless it
contains the Contractor’s Util
ity License Number on the
envelope. See Instruction to
Bidders for additional bidding
requirements.
1.5 PRE-BID CONFER
ENCE
A Pre-Bid Conference will
be held at 10 AM on Sep
tember 16, 2016, City Hall
Conference Room, 25 East
Midland Avenue, Winder, GA
30680. After a brief project
review meeting, Bidders may
drive to the Fort Yargo Raw
Water Pump Station for a site
visit. Bidders on Contract B
may look at that project on
their own.
Representatives of Owner
and Engineer will be present
to discuss the project. Bid
ders are strongly encouraged
to attend and participate in the
conference, but attendance is
not mandatory. Engineer will
transmit to all prospective Bid
ders of record such Addenda
as Engineer considers neces
sary in response to questions
arising at the conference. Oral
statements may not be relied
upon and will not be binding or
legally effective.
1.6 PRE-QUALIFICATIONS
OF BIDDERS
Prequalification of Bidders
for both Contract A and B
work is required. A copy of
the application and qualifica
tion requirements is included
as part of the Contract Docu
ments in Section 00471 and
may be obtained from the
Engineer by contacting Heath
er Cash at (706) 769-4119 or
hcash@cartersloope.com.
Completed applications
for pre-qualification for bid
ding (Section 00471) will be
received at the offices of the
Engineer until 2:00 p.m., local
time, September 8, 2016. All
applicants submitting pre
qualification documentation
will be notified of their sta
tus by 5:00 PM September
9, 2016. Only Bids submitted
by properly pre-qualified Bid
ders will be opened at the bid
opening.
1.7 BONDS
Bids shall be accompa
nied by a bid bond or certified
cashier’s check in an amount
not less than 10% of the base
bid. All bonds shall be by a
surety company licensed in
Georgia with an “A” minimum
rating of performance and a
financial strength of at least
five (5) times the contract price
as listed in the most current
publication of “Best’s Key Rat
ing Guide Property Liability”.
Performance and Payment
Bonds, each in an amount
equal to 100% of the contract
price shall be required of the
successful bidder if contract is
awarded. Each Bond shall be
accompanied by a “Power of
Attorney” authorizing the attor
ney-in-fact to bind the surety
and certified to include the
date of the bond.

Georgia law provides that
you can permanently lose
your rights as a parent. A
Petition to Terminate Parental
Rights (“Petition”) has been
filed requesting the Court to
terminate your parental rights
to your above-named child. If
you fail to appear, the Court
can terminate your rights in
your absence.
If the Court at the trial finds
that the facts set out in the
Petition and Amended Petition
are true and that termination
of your rights will serve the
best interests of your child,
the Court can enter a judg
ment ending your rights to
your child.
As the alleged biological
father that is not the legal
father, you are advised that
the Court can summarily termi
nate your rights to the above-
named child and you shall
have no right to object to such
termination unless within thirty
(30) days of receipt of notice
you file a petition to legitimate
the child and file Notice of the
filing of the petition to legiti
mate with the Barrow County
Juvenile Court.
If the judgment terminates
your parental rights, you will
no longer have any rights to
your child. This means that
you will not have the right
to visit, contact, or have cus
tody of your child or make
any decisions affecting your
child or your child’s earnings
or property. Your child will be
legally freed to be adopted by
someone else.
Even if your parental rights
are terminated: (1) You will still
be responsible for providing
financial support (child support
payments) for your child’s care
unless and until your child is
adopted; and (2) Your child
can still inherit from you unless
and until your child is adopted.
This is a very serious mat
ter. You should contact an
attorney immediately so that
you can be prepared for the
court hearing. You have the
right to hire an attorney and to
have him or her represent you.
If you cannot afford to hire an
attorney, the Court will appoint
an attorney if the Court finds
that you are an indigent per
son. Whether or not you
decide to hire an attorney, you
have the right to attend the
hearing of your case, to call
witnesses on your behalf, and
to question those witnesses
brought against you.
