National Type One diabetes
study available at MCG

at high risk for developing this
disease and then helps them pre
vent it.

across the country who have Type
'One diabetes in their family and
-are showing early symptoms of
'the disease themselves.
“It’s a wonderful opportunity
for the family to prevent a condi
tion which has long-term com
‘plications,” said Dr. William H.
Hoffman, pediatric endocrinolo
gist and principal investigator
on the study.
Type One diabetes typically
surfaces before age 40; Dr.
‘Hoffman has found it in new
‘borns. It tends to run in families,
-although doctors believe the com
bination of a genetic predisposi
tion and some trigger, such as a
wviral infection, may activate the
disease. When it does, patients
‘begin to feel “pokey,” Dr. Hoffman
'said, as the pancreas, which pro
‘ducesinsulinthat allows the body
to use food as fuel, becomes the
target of the body’s immune sys

worldwide organization.

formed the first People First
chapter in Atlanta. Now People
First of Georgia boasts over 15
chapters and 200 self-advocate
members throughout the state.

down and blood sugar levels go
up. At its worst, the body begins
to break down fat to use as fuel.
The disease can damage the eyes,
kidneys and cardiovascular sys
tem.
Inthisnational study, sponsored
primarily by the National Insti
tutes of Health, first-degree rela
tives, ages four to 45, of patients
who already have Type One dia
betes, will be screened for the pres
ence of islet cell antibodies, an
early indicator that the immune
system has begun to affect the
pancreas. For some reason, sever
al antigens, or proteins, normally
present in the pancreas become
targets of the immune system in
diabetes; the immune system’s
production of islet cell antibodies
is a by-product. “When the anti
bodies are present, you may have
the genetic predisposition to de
velop Type One diabetes and now
that genetic predisposition is be
ginning to manifest itself,” Dr.
Hoffman said.
Ifantibodies are found, partic
ipants will get a follow-up test
that looks more specifically at
how the pancreas is functioning.

the patient gets entered into a

The prevention program is de
signed to determine if a regimen
of insulin, given orally or by in
jection, can halt the development
of the disease. “If you don’t yet
have diabetes, but dohave mark
ers that you might develop it, the
exposure of the body to insulin
appears to protect the pancreag
so the immune system won’t tar
get it for further destruction.
“Experimental data with ani
mal models genetically predis
posed to develop Type One dia
betes shows you can prevent the
genetic predisposition from
evolving into the disease,” Dr.
Hoffman said. This study will be
thefirstlarge, randomized study
in which peopleidentified at high
risk will be followed for several
years to determine whether in
sulin can prevent disease devel
opment. Dr. Hoffman empha
sized that participation requires
a long-term commitment.
