AGENCY SURVEY FORM
The Augusta-Richmond County Commission through its Housing and Neighborhood Develop
ment Department is in the process of conducting a survey of agencies in Augusta-Richmond
County (ARC). The purpose of the survey is to identify agencies in ARC that provide services,
housing, job training, jobs, health care, etc. to LOW INCOME persons, families, youths, persons
with special needs, and public housing residents. The information collected through the
survey will be used to assist ARC in developing its Year 2000 five-year Consolidated Plan. The
Consolidated Plan identifies housing, homeless, community and economic development needs
and resources in ARC and tailors a strategic plan for meeting those needs. The development of
this Consolidated Plan is a prerequisite for receipt of Community Development Block Grant,
HOME and Emergency Shelter Grant funds. Agencies that can assist ARC in meeting the needs
to be identified in the plan may be considered for funding throughout the five-year plan
period. Therefore, please assist ARC by completing the survey form and submitting it to the
Housing & Neighborhood Development Department by Friday, March 19, 1999.
