TO PROCURE
The Northeast Georgia MHMRSA Regional
Board intends to make available to Interested
providers a Request for Information (RFI) docu
ment which seeks mental health and substance
abuse services provision to children and ado
lescents through the Medicaid Community
Mental Health Center Program.
The purpose of this Request for Information
(RFI) is to provide interested groups and orga
nizations with sufficient information to enable
them to prepare and submit responses tor con
sideration by the Regional MHMRSA Boards. It
provides general Information for the develop
ment and submission of responses along with
the description of services solicited.
The Northeast Georgia Regional MHMRSA
Board is issuing this RFI in order to identify an
additional children/adolescent provider to offer
Medicaid Community Mental Health Program
services as certified Medicaid providers These
providers will be selected by Regional MHMR
SA Boards and will be recommended to the
Department of Community Health's Division of
Medical Assistance as eligible providers to
whom that Division should offer a Medicaid
Provider Agreement.
Through this announcement, the Regional
Board wishes to create a mailing list for the dis
tribution of the RFI document Because the
Departments and Boards are Interested in
expanding the provider pool for public behav
ioral health services, offerors under this RFI
may not Include Community Service Boards,
since Community Service Boards are already
Medicaid-funded Community Mental Health
Program providers.
If your agency has an Interest In receiving a
copy of this RFI, you must submit a written

Authority and operated by Forsyth County,
Georgia (the ‘County’), (b) a debt service
reserve fund (or purchasing a debt service
reserve surety bond) for the Final Series 2000
Bonds and (c) the costs of issuing the Final
Series 2000 Bonds.
In such proceeding, the Court will also pass
upon the validity and enforceability of a 'Bond
Resolution' and a Lease Contract, dated as of
July 1, 1998 (the 'Lease”), between the
Authority, as lessor, and Forsyth County,
Georgia (the “County”), as lessee, as amend
ed. Under the terms of the Lease, the County
will (a) manage the water and sewerage
System, (b) pay the reasonable and necessary
costs of operating, maintaining and repairing
the System, including the cost of purchased
water under the Cumming Agreements (here
inafter defined) and other agreements, (c)
make payments to the Authority sufficient to
enable the Authority to pay the debt service on
the Final Series 2000 Bonds as the same
become due and payable (the 'Lease
Payments”) and (d) levy an ad valorem proper
ty tax on all taxable property located within a
special tax district (consisting of all of unincor
