died from blunt impact
injuries to his torso. He
had been left at home in
the care of his mother’s
boyfriend while she was
at work. The boyfriend
was arrested and in
dicted for murder and
related charges. His case
is pending trial.
A thirteen-year-old
white female died in
March from injuries sus
tained in a motor vehicle
collision. She was seated
in the front passenger
seat of an Acura, wear
ing a seat belt. A pickup
truck struck the passen
ger side of the car when
it turned left in front of
the truck.
Investigation revealed
the car had been in
volved in three other
collisions, one of which
totaled it. All occurred
prior to the car’s pur
chase by the driver's
family, which was also
unaware of the history.
Although the officer
determined the car’s
driver failed to yield, no
charges were brought
because he was of the
opinion that the previ
ous damage to the car
contributed to the seri
ousness of the collision.
The committee recom
mended stricter regula
tions related to rebuilt
vehicles and a prohibi
tion against the resale of
rebuilt totaled vehicles.
Also in March, an eleven
year-old Hispanic male
died of acute lympho
blastic leukemia. He was
diagnosed with a treat
able form of leukemia in
July 0f2012; however, his
mother failed to comply
with doctors’ orders re
lated to his treatment
regimen. Members of |
the school system, CASA
and DFCS were con
cerned that the mother’s
neglect, as well as the
coutl’s delay in remov- |
ing him from her custody
had caused his relapse |
and death. ‘
The committee, includ
ing the representatives
from the agencies diract
ly involved in his care,
agreed that better com
munication was needed
among all involved in his
case. Everyone |
also acknowledged that
this could be better ac
complished if the same
agency representative
remained for the pen
dency of the case. The |
length of time to resolve
this case resulted in
turnover among those
assigned to it. Prior to |
the committee’s review,
the school system be
gan pursuing the pros
ecution of educational
neglect cases in State
Court and CASA created
a referral form for cases
that do not involve foster
care in efforts to prevent
a situation as occurred
here. CASA also joined
the review committee.
In April, a one-year-old
white male died of Spi
nal Muscular Atrophy.
He had been previously
diagnosed and given
a life expectancy of six
months. After participa
tion in a clinical study, his
condition had improved.
The committee encour
aged continued medical
research.
In May, a seventeen
year-old white male
hanged himself in a re
stroom while attending
a concert outside the
county, The manner of
death was determined
to be a suicide based on
his history, blood alcohol
level and statements
made by an individual
in attendance at the
event with decedentand |
others. The committee
recommended stricter |
parental supervision as a
‘preventive measure. |
In June, a seventeen
year-old white male died
from injuries sustained
during a One vehicle col
lision. A truck, driven
by a seventeen-year-old
male, struck a pole and
the decedent, unre
strained, was ejected.
The occupants, includ
ing two other teenagers,
had been drinking. The
driver had previously
been charged with un
derage possession of al
cohol. He pled guilty to
vehicular homicide and
was sentenced to a pris
on term. The committee
recommerided that the
administrative proce
dures (referred to as
1205 hearings) used for
the restriction of drivers’
licenses for certain dru?
and alcohol-related of
fenses should be ex
panded to include other
drug and alcohol-related
crimes, incMiding under
age possession of alco
hol, when committed by
juveniles.
A ten-year-old Asian In
dian male died in August
from endocarditis due to
congenital aortic valve
stenosis, with which he
had been diagnosed at

THE FORSYTH COUNTY
CHILD FATALITY REVIEW
COMMITTEE
FOR THE CALENDAR
YEAR 2014
As mandated by Georgia
law (O.CGA. §l9-15-3
(k)), the Forsyth County
Child Fatality Review
Committee submits this
report to the Georgia
Child Fatality Review
Panel and for publication
in the Forsyth County
News in July and De
cember. The committee
received notification of
twenty-four deaths of
Forsyth County children
that occurred during
2014. Determining that
fourteen met the criteria
for review, the commit
tee conducted meet
ings on these cases and
submitted reports to the
State Child Fatality Re
view Panel.
These deaths met the
statutory criteria for re
view because they oc
curred to children from
birth through seventeen
years as a result of Sud
den Infant Death Syn
drome, unexpected or
unexplained conditions,
injuries (intentional and
unintentional), sudden
death when the child
was in apparent good
health, a suspicious or
unusual manner, medi
cal conditions while un
attended by a physician
or while the child was
incarcerated in a state
hospital or state, county
or city penal institution.
The reviews included
inquiries into the cir
cumstances that led to
the deaths; the extent
government agencies
had been involved with
the children and their
families, as well as de
terminations as to the
preventability of the
deaths and whether any
action could be taken by
the local community or
at the state level to pre
vent similar deaths. The
cases reviewed are sum
marized below.
In February, a four-year
old white male strangled
himself with a blind cord
in his
bedroom while his
mother and siblings
were in their own rooms.
It appeared from the
investigation that the
child may have been at
tempting to jump from a
nightstand or windowsill
to his bed and slipped.
The committee deter
mined that the death
could have been pre
vented by the use of
breakaway blinds or
trimming the cords. It
recommended that this
information could be
conveyed to parents by
daycares and pediatri
cians. DFCS currently
requires all foster homes
that have window blinds
to use the breakaway
kind.

year-old white male

« A set-up fee equal to the cost of a single insertion will be
assessed on any foreclosure cancelled after receipt and
birth. Complaining of
stomach pains, he went
to see a physician to get
clearance for testing. He
also experienced trouble
breathing and had leg
pain. Be vomited be
fore seeing the doctor
and was sent home. His
parents were told the
clearance documenta
tion would be sent to
them. His parents then
took him to the pedia
trician. He was given
medication and sent
home. He fell asleep
while on a sofa and his
mother remained with
him through the night.
He was found unrespon
sive the next morning.
The committee believed
the death was prevent
able. There was some
question as to whether
the pediatrician gave
sufficient consideration
to the childs history
and also, if he should
have been taken to the
hospital, rather than the
doctor’s office. The com
mittee also expressed
concern that clearance
for the testing was go
ing to be approved even
though the child was not
examined.
Also in August, a two
month old white male
child was discovered
deceased by his mother.
She had placed him in
her bed the previous
evening on a decora
tive pillow and pulled a
comforter over him. She |
also slept in the bed. He |
slept through the night. |
She awoke in the morn- |
ing and found he was
not responsive. He had
a crib in another room,
which was observed |
during the investiga
tion to contain various |
items. The medical ex
aminer determined the |
cause of death was SUID |
(sudden unexplained |
infant death) “with bed- |
sharing /prone sleeping |
position/unsafe sleeping |
condition” and the man
ner was undetermined. |
A sixteen-year-old white
female hanged herself |
at home in September. |
She was upset at being |
directed by her parents |
to end a romantic rela
tionship. She was taking |
medication for anxiety |
and treating with a psy- |
chiatrist and therapist. |
A seventeen-year-old |
white male also died in
September after experi- |
encing what appeared |
to be a seizure while at
his home. He had been |
diagnosed with a sei
zure disorder at the age
of eight and was taking
medication for it at the
time of his death. The
cause of death was de
termined to be the dis
order.
in October, a two-month
old white male died from
positional asphyxia and
the manner of death was
ruled an accident. The
~ child usually slept in a
~ crib in a room next to his
- mother’s; however, he
occasionally slept with
her, as on this occasion.
His mother fed him in
the early morring hours
and they both went back
to sleep. She woke a few
hours later, on top of
him. He was nonrespon
sive and resuscitation ef
forts were unsuccessful.
A white male, six weeks
old, died in November.
The mother initially re
lated that she
breastfed him on the
sofa in the early morning
hours. She then put him i
back in a baby swing,
placed near the sofa,
where she remained,
They both returned to |
sleep. She found him |
nonresponsive when
she went to remove him
from the swing a few |
hours later. A pattern
on the baby’s right thigh |
was observed to be con- |
sistent with a pattern on
the sofa, indicatirg the
child died while there,
before being placed
back in the swing. ‘
When presented with
this information by law
enforcement, the moth
er admitted that she
briefly fell asleep while |
nursing and maintained
that the baby was alive
when she returned him
to the swing. The medi
cal examiner ruled that
the cause and manner
of death were undeter
mined, but SIDS and
overlay were possibili
ties. No charges were
filed.
In December, a twenty
three-month old white
female died from un
specified natural causes. *
After developing a fever |
in the morning, she was
given medication. Her
older sister had recently
been diagnosed with
the flu. She was placed
in her crib in the evening
and found nonrespon
sive when her mother
looked in on her several
hours later.
Based on the deaths |
from this year and the
preceding year involving |
children sleeping with |
their parents and in adult |
beds with bedding, the |
committee expressed |
concern about whether ’
educational effolts that |
have been routine on |
this issue for some time,
were still being under- |
taken and, if so, were |
beillg heeded. Based |
on the circumstances in |
these cases, including
the presence of cribs and |
young ages of the moth- |
ers, the committee also
expressed concern that
the advice was being
ignored.
The Coroner met with
an official at the local
hospital to confirm that
new parents were be
ing informed about safe
sleeping. The local Safe
Kids chapter also rou
tinely informs parents
about safe sleep colldi
tions when providing car
seat inspections and can
provide Pack 'n Play cribs
for infants in need.
