Acne and facial hair are more than skin deep
BY PERRY WELLS, M.D.
D ealing with acne is
a stressful and frus
trating problem for
far too many young
patients. At least for
female patients, the problem is al
most always tied to underlying hor
monal effects. Trying to treat the
surface without going after the root
of the problem may give disap
pointing results.
The skin has little structures
called pilosebaceous units,
which, in some areas of the
body, are highly sensitive to
male hormones, also called
androgens. Wait you say, “I
thought we were talking about
females.” Well
women make male
hormones too; just
less of them. In some
instances, such as
with a very common
disorder called poly
cystic ovaries, there
may be enough extra
androgens to over
stimulate the pilose
baceous unit result
ing in acne and some
times increased facial hair.
As a matter of fact, women
who have problems with acne
often also have problems with
facial hair, since the
underlying hormonal
disorder is the same.
Actually more women
have a problem with
unwanted hair around
the chin, lips, and
abdomen than have
acne. This unwanted
hair growth, like acne,
WELLS is less likely to
respond to surface
treatments if the hor
monal influences are not
addressed.
In other cases, there is not
too much male hormone, but
there is over activity of an
enzyme in the skin which con
verts the hormones into a very
potent androgen. This andro
gen called dihydrotestosterone
is about ten times more potent
on the sebaceous glands and
hair follicles. The bad hor
mones can be either sup
pressed or blocked by a variety
of well tolerated medicines. In
my experience, this internal
approach is more effective
than just treating the surface.
At any rate, there are effec
tive ways to decrease the
amount androgens produced or
to block their effect on the
skin. So, for women whose
lives are being made a little
more complicated by those
pesky pimples and hair folli
cles, it might be time to think
of the problem as being more
than skin deep.
