Notice is hereby given that
effective March 7, 2010, the
agreement between Cobb
Health Care Center, 2430
Paoli Road, Comer, Georgia
and the Secretary of Health
and Human Services, as pro
vider of nursing services in
the Health Insurance for the
Aged and Disabled Program
(Medicare), is to be terminat
ed.
The Centers for Medicare
and Medicaid Services has
determined that Cobb Health
Care Center is not in compli
ance with the Requirements
for Participation. The Medicare
program will not make pay
ment for inpatient nursing
services to residents who are
admitted after March 7, 2010.
For residents admitted prior to
March 7, 2010, payment may
continue for a maximum of
30 days for nursing services.
Such payment is specifically
limited to covered services
through the close of business
April 7, 2010.

The conduct giving rise to the
forfeiture is as follows: On
01-26-2009 a search warrant
was conducted at the resi
dence of James Sims Jr. Sims
was found to be in possession
of several baggies containing
an off white crystal like sub
stance that field tested positive
for methamphetamine. Sims
was also found to be in pos
session of several pre-pack
aged baggies of a green leafy
material suspected to be mari
juana. A quantity of pills and
unused baggies and scales
were also found. Pursuant
to Official Code of Georgia
Annotated Section 16-13-49(n)
this property is subject to for
feiture. Any person claiming
an ownership interest in this
property may file a claim within
thirty (30) days after the sec
ond publication of this notice
by sending the claim to the
seizing agency and to the dis
trict attorney by certified mail
or statutory overnight deliv
ery, return receipt requested.

be forfeited and disposed of
