2005, at 9:30 a.m., in the Juvenile
Court of Houston County, 202 Cart
Vinson Parkway, Warner Robins,
Georgia 31088.
This notice is provided to you pur
suant to O.C.G. A. Section 15-11-96(e).
You are the alleged biological father of
the child at issue in this matter, and
you have not surrendered your
parental rights to this child, to the
knowledge of the petitioner. This is to
notify you, pursuant to O.C.G.A.
Section 15-11-96(h), that you will lose
all rights to the child at issue in this
action and you will not be entitled to
object to the termination of your rights
to the child unless, within 30 days of
your receipt of this notice you file:
(1) A petition to legitimate the child
pursuant to O.C.G.A. Section 19-7-22;
and
(2) Notice of the filing of the petition
to legitimate with the Court in which the
action under this code section is pend
ing, which is the Juvenile Court of
Houston County, Georgia.
A biological father who is not the
legal father loses all rights to the child
and the Court shall enter an Order ter
minating all such father’s rights to the
child and such father may not there
after object to the termination of his
rights to the child if within 30 days from
his receipt of the notice provided for in
O.C.G.A. Section 15-11-96(e), he:
(1) Does not file a legitimation peti
tion and give notice as required in
O.C.G.A. Section
(2) Files a legitimation petition which
is subsequently dismissed for failure to
prosecute; or
(3) Files a legitimation petition and
the action is subsequently concluded
without a Court order declaring a find
ing that he is the legal father of the
child.
