With 52 clubs in the state
and 160 members state
wide, Georgia is the larg

Exchange Club. According
to Middlebrooks, recruit

civic club because it is the
club members who get the
job done. “Volunteerism is
declining,” he said.
“The young people of today
are busy with other things
and don’t find the time to

What is proper dosing?
One of the more fre
quent questions
asked of pharma
cists is “When should I take
this medication?” And this
can be a very important
question to ask because the
effectiveness of some med
ications is changed when
they are taken at different
times of the day.
Some medications are
even designed by the manu
facturers for dosing at a
specific time of day. Covera
HS and Verelan PM, for
example, are formulated to
be taken at bedtime. These
medications are formulated
to have a maximum con
centration of medication
in the blood in the morn
ing hours, which is when
rapid increases in blood
pressure may occur in some
patients.
Some other examples of
medications that may work
better when taken in the
evening hours include the
cholesterol lowering drugs
Mevacor, Zocor, and Lescol;
the asthma medication,
Singulair; the Alzheimer’s
medication, Aricept; and
stomach ulcer/acid reflux
medications such as Zantac,
Nexium, Prilosec and
Protonix.
The above mentioned
cholesterol lowering medi
cations work better with
evening dosing because
most cholesterol produc
tion in the body takes place
at night. These particular
medications have short
er periods of activity and
should be dosed at night so
that the time that the maxi
mum amount of medication
is in the body coincides with
the time that cholesterol
production is at its peak
However, there are other,
newer, cholesterol lower
ing medications (Lipitor,
Crestor and Pravachol, for
example) that have a longer
duration of activity and can
be taken at any time of the
day.
The medication Singulair
should be dosed at night for
maximum efficiency when
being used to treat asthma,
however, when it is used

for general allergies, it can
be taken at any time of the
day.
The reason for this has
to due with one of the char
acteristics of asthma. In
patients with asthma, the
airways in their lungs tend
to be at their most narrow
during the early morning
hours. Taking Singulair at
night helps keep those air
ways more open during this
time, lessening the likeli
hood of an attack.
When a person who does
not have asthma takes
Singulair for seasonal aller
gy relief, narrowing of the
airways does not usually
occur, so evening dosing is
not necessary. It is impor
tant (as it is with all medi
cations prescribed as once a
day) for this medication to
be taken at the same time
each day. This ensures con
sistent blood levels of medi
cation and a better overall
response.
As with airway narrow
ing in asthma patients, acid
production by the stomach
in acid reflux/ulcer patients
can increase during the
night. The times when the
acid levels in the stomach
peak are usually between
2:00 and 4:00 AM, due to
the fact the stomach has
usually been empty the lon
gest during this time peri
od.
During the day, food is
regularly consumed, which
tends to neutralize acid in
the stomach. Many patients
with chronic heartburn
(which can be serious and
should be treated by a phy
sician) find that frequent
snacks during the day often
ease their discomfort. So
Houston, Bibb and the
Georgia State Patrol are
involved, Bame said, it
allows the different agen
cies to talk directly with
one another.
“We’re all on different
frequencies now.”

when the stomach has been
empty for a while, acid
levels in the stomach can
reach high levels and irri
tate the lining of the stom
ach; so much so that the
pain can awaken a person
from sleep.
In some instances, people
may be taking these medi
cations during the day, but
still having problems at
night. Moving the dose to
bedtime may be of some
benefit, however, if symp
toms persist, notify your
physician immediately.
Lastly, I want to point
out that this is just a brief
overview of a few medica
tions that should be taken
in the evening. There are
more and there are many
medications that should be
taken in the morning.
My point is this; make
sure you know when you
should take each your
medications. If your direc
tions are not clear or if
you want to know why a
medicine should be taken
at a certain time, ask your
pharmacist. Because of all
the things pharmacists dis
pense, information is often
the most important.
