Broken bones difficult to spot without x-ray

Several principles are valuable
when dealing with broken bones. First
of all it is important to recognize that
a broken bone is present or likely to be
present in a given injury. Some frac

feeling ambivalent.
Every holiday function seems to
be surrounded by forbidden foods.
Schedules for self-management
can be disrupted by special events.
Stress can increase due to family
pressures, time constraints, and fi
nancial obligations. Plus the end of
the year is routinely a time for indi
viduals to reflect on the successes
and failures of the past. This often
leads to the realization that diabe
tes will continue to have to be faced
in the years to come.
The person with diabetes and
their family may alternate be
tween feelings of denial, fear, an
ger, guilt, and depression. These
feelings are perfectly natural un
less they continue for extended pe
riods of time and become so intense
that normal daily activities are
disrupted.
In most instances the individual
and their family comes to some de
gree of acceptance of the disease.

shape or appearance of an arm, leg,
finger or facial structure. Others are
less easily identified. When deformity
is not present, fractures may be sus
pected if there is swelling and/or blu
ish discoloration beneath the skin.
Great tenderness at the site of inju
ry may also represent a broken bone.

negative feelings may occasionally
return.
If changing how an individual
feels about diabetes is difficult, the
best option is to change how one
expresses u-at feeling. The friends
and family of the person with dia
betes can be helpful in planning ac
tivities that can short-circut
depression.
Activites such as shopping trips,
outings to see holiday decorations,
gift wrapping get-togethers, visits
to shut-in friends or relatives, holi
day concerts, and excursions to
take those forbidden gifts of food
from well-meaning friends to hos
pitals and nursing homes can all
lift drooping holiday spirits.
The holiday time is a golden op
portunity to change a possibly diffi
cult season into a time of generos
ity and warmth toward others.

ment of an injured body part indicates
that there is no fracture present this is
not true. Though pain from a broken
bone may inhibit movement it is pos
sible to have a fracture present with
little or no effect cm movement. In
fact, I have seen one patient who
walked into the Emergency Depart
ment on a broken hip which required
surgical repair the following day.
With some types of injuries it is the
best policy to assume that a fracture
is present until x-ray can prove that
there is none. This is especially im
portant with injuries which may in
volve the spine. The mechanism of
these injuries may be a motor vehicle
accident, a fall, or a sporting acci
dent, among others. Any head injury
sufficient to cause unconsciousness
may also produce neck injury (up to
50 percent of the time in some stud
ies). Any accident victim who has dif
ficulty moving arms or legs or who
complains of tingling or numbness in
the limbs should be suspected of hav
ing a neck or back fracture. It is ex
tremely important that these patients
not be moved except by trained emer
gency medical personnel.
Once a fracture is identified or
strongly suspected there are several
measures which may help to relieve
the victim’s pain and limit the dam
age done by the injury. Any broken
limb should be splinted in order to sta
bilize the bone fragments. Movement
of one piece of bone against another
causes extreme pain. An effective
splint can prevent this and can also
stop jagged bone fragments from
damaging blood vessels, nerves and
other soft tissues. Any hard flat object
such as a board, a book or a folded
piece of cardboard can be adapttl to
serve as a spli.it. Ideally the broken
limb should be bound to the splint with
tape, gauze, or strips of cloth. Howev
er, it may be simply held in place by
hand if these items are not present.
The splint should be kept in place
while the patient is transported to
your doctor’s office or to the Emer

gency Department. An ice pack ap
plied to the area of fracture may help
to diminish pain and swelling and ele
vation of the limb whenever possible
will help as well. It is not necessary,
nor advisable to try to straighten the
area of deformity as this movement of
bone fragments may damage soft tis
sue structures. This treatment is best
left to your doctor who is trained to
recognize blood vessel and nerve
injuries.
When neck or back fracture is sus
pected every effort should be made
not to move the victim unless abso
lutely necessary. In those cases
where a patient must be moved (for
example in auto accident with a fire)
those supplying first aid should at
tempt to hold the head stationary with
regard to the body while the move
ment is carried out. When a patient
with suspected neck fracture is safe
from further accidental injury efforts
should be made to stabilize the neck
by arranging rolled clothing or other
objects on either side of the head.
In some fractures damage to near
by tissues is more important than the
broken bone itself. This is often true in
rib fractures. Thee is usually very lit
tle which needs to be done for a rib
fracture per se. However, the injury
which produced it may also have
bruised or punctured the lung be
neath. Bruises to the heart are also
possible and lacerations to the spleen
or liver sometimes accompany frac
tured ribs. Therefore, it is important
to seek your doctors attention if you
suspect a fractured rib.
Skull fractures may also be associ
ated with severe internal injuries,
some skull fractures are obvious but
the majority can not be identified by
outside observation.
