|'TN these days of surgical exploration and X-ray
'I corroboration a good many queer things are dis
covered about the inside works of the ever mys
terious human machine. For instance X-ray photog
'nphy has taught us that when a patient places his
hand upon his stomach and proceeds to slander that
hard-working organ there is a falr chance that the
stomach may not be there at all, but down visiting the
appendix or away on a junketing trip somewhere
plong the alimentary canal.
, Among the most curious disclosures of X-ray
photography is the fact that some peopls have an
extra palr of ribs in the neck where ribs have no
right to be. And in the past these individuals have
been permitted to suffer for years with what was
called “neuritis,” “neuralgia” or even aneurism, be
cause the symptoms caused by cervical ribs closely
simulate those of the conditions named.
Although there is gemerally a pair of these ribs,
the symptoms complained of are usually limited to
one side, And these symploms vary considerably
in different cases, the only constant complaint being
intermiitent or continuous pain, numbness, coldness
or cramps in one arm over a period of years. The
mere fact of long duration iy againet the diagnosis of
neuritis, because neuritis rarely continues for years.

most cases not
until after the twentieth year.
Women, somehow, seem to have & monopoly
on cervical ribs, about nine out of every ten cases
noted being among members of the sex which is sald
to owe soo much to one of the original thoracic ribs
of man.
The omsét of the trouble is very gradual, without
epparent exciting cause. But sometimes colncidences
are assumed to be causes of the symptoms of cervical
rib. For example, in one case an attack of typhoid
fever had preceded the paln and was supposed to
have left a “neuritis” as an aftereffect. In another
case the woman had taken a tramp in the woods on a
cold day and experienced the first paln In her arm
soon afterward—thus, to her own satisfaction at least,
establishing the well-worn, but nowadays rather dud-

We have stormed mostly in vain
against the vulgar propensity to
attribute every fllness to “taking
oold,” but hers we have s pretty
good case, at last. It is difficult
to imagine that “exposure™ could
cause a rib to grow in a lady’s neck
over night!
In all cases of cervical rib the
two factors most responsible for
attacks of pain are cold and the
use of the arm. The paln sal

In every instance of cervical rib
there is & tendency for the symp
toms to besome more severs and
persistent.
The pain is usually described as
dull in character, and may be con
fined to the forearm or felt in the
upper arm, shoulder or between
the shoulders.
Numbness and tingling of the
fingers is frequent even when
there is no pain present.
Some patients have severs
eramping of the fingers after carry
ing even a light package in the
hand. One patient had such
eramps for ten years before aoute
pain developed. The cramp was
mnot painful, but the patient had to
use her other hand to straighten out
the fingers after a cramp—a very suggestive symp
tom of cervical rib.
In ell patienis ile arm tires quickly. Many suffer
only from fatigue or numbness when Writing or sew
ing or doing other light work with the hand, which
disappears after a few minutes rest.
As & rule blanching or blueness of the hand or
arm is observed after moderate exposure to cold. In
one patient the hand and arm was continually pale
and would not redden appreciably even when placed
in hot water. In others the har~4 was noticeably blue,
contrasting strikingly with the normal color of the
other arm. A nurse with a cervical rib achieved quite
an unenviable reputation for her one cold hand.
Unlike neuritis of the brachial plexus (the great
nerve plexus which supplies the arm), cervical rib
causes no marked atrophy or wasting of muscies and
skin. In neuritis of any severity there is always
thinning and shiny esppearance of the skin, and
shrinking or wasting of the muscles Involved.

It is difficult, usually impossible
to feel the superfluous rib in the
neck.
The ocervical rib may be very
short. In one case it proved only
half an inch long, and for that rea
son had been overlooked by a for
mer X-ray examiner as & cause of
the patignt’s trouble. Yet its
surgical removal brdught prompt
andl permanent relief.
The X-ray photograph in possi
ble cases of oervical rib must be
interpreted by an expert. The un
experienced examiner may mistake
& oervical rib for an unusually long
transverse process on a oervical
vertebra. BSometimes the oarvical
rib ourves sharply forward apd in-

visage we shall grow to think of
ourselves as gloomy or irritable,
and will become so,
