father that you lose all rights to the
child and will not be entitled to ob
ject to the termination of your paren

thirty (30) days of the receipt of this
notice, you file: (1) A petition to le
gitimate the child pursuant to
O.C.G.A. Section 19-7-22; and (2)
Notice of the filing of the petition to
legitimate with the court in which the
action under this Code Section is
pending.

be obtained from the Clerk of the Ju
venile Court at Pulaski County
Courthouse, Pulaski County, Geor
gia 31055, during regular business
hours, Monday through Friday, 9:00
a.m. until 5:00 p.m. exclusive of holi

copy will be mailed to the requester.

custody of the Pulaski County De
partment of Family and Children
Services.

and convincing evidence of parental
misconduct or inability indicating
