required: Pneumococcal Conjugate (PCV) for
daycare. Headstart and preschool only and
Hepatitis A, for all children bom after Jan. 1,
2006, officials note.
Additional doses have also been added to
two existing vaccines requirements for all
children entering kindergarten, sixth grade
or new entrants (children of any age entering
Georgia schools for the first time or return
ing to Georgia schools after being out of the
system for a year). These are: MMR (measles,

to four doses; MMR (measles, mumps and
rubella) - two doses; Varicella (chickenpox) -

Children can receive up to five vaccines per
visit and dosage requirements vary by age,
according to officials.
The health department also offers hearing,
vision and dental screenings (Form 3300)
required by schools, as well as Immunizations
and Certificate 3231. Before coming to the
health department, please obtain immunization
records from your child’s pediatrician, and if
not in English, please have them translated,
officials said. Data entry fees will apply if
the information has not been entered into the
Georgia Registry of Immunization Transactions
and Services. Payment methods accepted
include Medicaid, Peachcare, cash, or check,
officials said. Insurance will not be filed by
the health department, officials added. Call the
clinic at 706-795-2131 for more information.
